Helping physicians prescribe more efficiently, accurately and safely

Typically, physicians are responsible for developing treatment plans for patients. These treatment plans often involve complex drug regimens, dose calculations, and the physician has to ensure individual dose and cumulative doses’ limits are not exceeded.

With BD Cato™**, physicians prescribe treatment plans using a framework of pre-built treatment protocols, with predefined drug and dose recommendations. Past chemotherapy drug/dosage history can be viewed, and due to BD Cato™ interfacing with the hospital’s electronic medical record system, patients’ laboratory results are readily available.

With BD Cato™, automatic notifications appear to inform the physician if they have prescribed a dose outside the predefined recommended limits. However, physicians retain the ability to confirm their decision if there is a clinical reason to do so - but then it is by choice, not error.

Clarity and priority in the pharmacy

BD Cato™ eliminates time spent deciphering handwritten orders as physicians prescribe the treatment plan electronically, which is then sent directly to pharmacy. Any updates to the treatment plan by the physician can be seen in real-time by pharmacy, helping to ensure effective communication.

BD Cato™ provides oversight of the workload within pharmacy, and automatically places the most urgent prescriptions first in the queue. This real-time oncology workload management injects high levels of efficiency into the process.

Inventory and dispensing management

BD Cato™ helps to avoid delays caused by stock-outs due to providing comprehensive inventory and dispensing management. It provides complete visibility and traceability of inventory through tracking the quantity, batch numbers and expiry dates of stock, as well part-used drugs remaining after compounding. BD Cato™ suggests the most suitable vials to assemble for each dose, helping to ensure part-used vials are used up first. This can result in a reduction of drug waste and therefore deliver cost savings.

Complementing these processes is BD’s closed system drug transfer device, BD PhaSeal™. One study looking at the impact of BD PhaSeal™ showed the pharmacy avoided nearly half of the potential waste and saved a mean of 29% of each vial. That represented an annual saving in pharmacy waste alone of $703,047.673.*

Avoiding human error in compounding

Drug compounding presents multiple opportunities for human error, which can lead to increased costs and increased risk to patient safety. On top of this, many of these errors are not even detectable during the manual compounding process. A large-scale European multi-center study assessed the impact of automation* and discovered that of 759,060 chemotherapy preparations, the mean out-of-tolerance error rate during compounding was 10.44%.4 These errors were detected by the gravimetric technology. The study concluded that it is unlikely these errors would have been detected using a manual compounding process.

During compounding, BD Cato™ informs the user if there is an error, allowing it to be rectified immediately. A hardstop will prevent the user from continuing until the error has been corrected, helping to ensure patient safety. Once the dose has been correctly prepared, BD Cato™ will create a unique barcode for the final product. This barcode provides traceability of the prepared dose, helping to ensure the correct patient receives the correct drug.

When there has been a medication error, it can be time-consuming to identify when and where it originated. Was it a dosage error? Did the pharmacist misread the physician’s note? Did the physician over or under-dose the patient? Did the nurse select the wrong bag?
